Process chain not stating subsequent job

We are on NW04s and while process chains does not kick off next job even though the previous job completes sucessfully and the process is Green.

We do not use DTP process to load data.

is it a pattern or for the specific process chains .

As am on SP-09 and still works for 20 process chain I created ,Includes the Meta chain as well

Well here could be the catch..

Have a look below from the OSS -914305 (SP -11)

In very quick parallel asynchronous processes, even though one or more processes may be successful, the process chain log may not contain any instances. As a result, the chain does not continue after the successful process. Note 994007 causes this error. For further information, refer to Notes 1005481 and 1003941.
