Your layout might be wrong.

Check in the layout if you use repetitivearea or not. Item no. must be put in the repetitivearea.

I think the pld of draft document is wrong.

Another possible cause is the draft document contains 5 items but in actual document is only one item. User might have deleted 4 other items.
