I have a SC that's been approved by a manager via the offline method.
We schedule report RBBP_OFFLINE_EVAL to run every few minutes to process these offline approvals and create a spool list. In this spool list, I see one entry that says:
Work Item 000001234567
Work item agent SMTIH is locked. E-mail was not deleted. Work item was not executed.
Does anyone know what causes this lock? I check SM12 and I don't see anything there. How can I unlock this work item?

From the errro it seems that the user SMTIH is locked.
Please check in t-code SU01 if the user s locked.
Unlock the user (take basis help) and try again.

So do you mean that SMITH was the manager who approved the SC offline?
I do not know if my answer would be relevant,
pls check the following note 174936 and could check SWIE which is generally used for unlock workitems.

Please check the user is not locked in SU01.
Please also check the workitem status in SWI1.
