Customer statement smartforms in particular format

I am working on customer statement report.I created in classical format, its working fine( got the desire output ).But i want to do it in smartforms and attatching the desire format . Is it feasible in smartforms below output

Yes you can but you may not get the exact features of report in Smart forms. You may have to discuss with business and understand what they need in headers and how they would like to have the form.
