[BUG] Deleted items are not removed from Activity stream

It's very annoying when you try to open some item from Activity stream and result is:

Access Denied

We're sorry, but you do not have permission to do the activity you attempted. If you believe this to be in error, please contact the site administrator(s).

To my mind, deleted items have to be at least marked as "Deleted" in the activity stream.

Also changes are not updated in the activity stream (Former member <-> Real name)
