For UTP, certain Document formats will be there with Client's which all the developers have to follow. We need to give some inputs and check for proper outputs. This can be recorded in Tables in Word document. Sometimes the Screen shots are to be attached also.
Also the UTP should have the screen-shots of the Performance analysis like Run-TimeAnalysis and SQL Trace (ST05).
The UTP may be attested with signature of the End-User, the Functional consultant also.
