1099MISC Adobe form prints 2 per page

Hello Guru's,

I have implemented OSS Note 0002398337 successfully on our SAP7.31 system. But when I try to print with Adobe form IDWTCERT_US_1099MISC, it prints 2 per page. It works fine when I do a test print from SFP but when I run it from transaction S_P00_07000134 it prints 2 per page. Has anyone else encountered this issue and know how to fix?
