Request Rejected inspite of being approved

Hello All ,

I am facing this problem from last few days and have not been able to solve it .

There is an order (Unplanned Return ) whose approver is maintained as someone . But the request for approval goes to someone else and when he approves it , it gets rejected and the Status is "Rej "

Can anyone tell me what exactly is the problem and where ?
