Customer evaluation/Survey/Rating

we have set up a helpdesk with incidents/service requests etc.

Once an incident / service request is "resolved" an email is being send to the end-user who is informed on the solution. We would like to have the end user then complete an evaluation/survey/raing on how the call has been handled.

Any pointers to such utilities or how this can be done.
