OSA - Std Report with status and sub-status

Hi PM experts,

Do we have any standard report that we can use to pull the status and sub status of appraisal documents? I do see only status but didn't see any column for sub status in reporting tcodes - phap_admin, change etc.,

Did any one use appcreate/appchange tcodes? What is appraisal model?
