Document Locked, but not unlocked in change mode

When ever user is trying to change the documents (shipment/delivery/sales order) document is getting locked, but it is not getting unlocked..

In de-bugging i checked, DEQUEUE_ALL is getting triggered, but it is not unlocking the documents.
