ESI Contribution Period - Form 5

Hi,

While generating the standard Form 5 (ESI), the system is not displaying the contribution period (ie. Apr-Sept / Oct - Mar).

Also, is there any possiblity to increase the character length of employer reference code field........standard is 15...i want atleast 17....

pls suggest.
