Application Workflow - Routing to agents from previous step

Hello,

We are using application workflow in SRM 7.0. There are multiple levels of approvals. The approval work items for the first 3-4 levels of approvals are routing correctly. However after that the next level approval work item is being routed to the agents from the previous level (level 4/level 5 - sometimes it is happening at one level and sometimes at the other). And the workflow log in the backend is not showing any updates (any new work items/tasks that were generated). It is only showing the logs upto the level 4/level 5 - till the point where routing was correct. Appreciate any help on possible reasons for this issue.
