How to get the URL path in SAMRTFORM

Hi Experts,

How to send the URL path to smart form.I searched for a table but I did not find any table but I am able to see the path in T.code: SICF)

in my secnario i have to send the smartform as email notification through actions.Below url format is expected formated in the smartform.

For example the URL is:

http://cd1ci01.xyzcorp.com:8010/sap(bD1lbiZjPTIyMSZkPW1pbg==)/bc/bsp/sap/crm_ui_start/default.htm?crm-object-type=CRM_SRQM_INCIDENT&crm-object-action=B&crm-object-value=4CEE75914C912FFDE1000000C0A87F43

Please help me.
