In TEM what level is Busines Event Appraisal assigned - is it at the Business Event Type level?

No it is assigned to business event itself.

you can use PHAP_CREATE
Regrads,
T-code PV33 can be used for business event appraisals. As pointed out by Dilek, PHAP_CREATE can also be used if you are using the old Appraisal systems.

Actually PHAP_CATALOG is the t-code for the new Objective Setting & Appraisals system, if I am not mistaken.
