Upgrade from Vanilla SAP ECC 6.0

Good Evening BASIS experts!

Im about to roll-out on upgrades to a Naked SAP ECC 6.0 installation.

For me to Upgrade the instance into EHP 5, how many Support Packages are to be implemented before jumping on to the next EHP?

Are there SAP Documentations regarding this? I can't seem to find actual step by step procedures on how to do this.
