I need to create a from(Iview) with few input fields and two buttons.

first name

last name

date of birth

radio buttons for Gender

Buttons for

SAVE CANCEL

once user click on the SAVE button, the information should send to one email address.

I am new in this area, so can you please give the code for this. This is my first program in JAVA/JSP area. Please help me. if we need to use html or XML, then need XML.

I have to create around 15 form some what like this. so onece I get this, then I can do the rest. Please help me.

If you don't mind, can you please give the code. I can give my email address. So you can send into this email address - toms3501@yahoo.com. I want to do through Eclips which I downloaded into my system, and when everything working fine, I can Export the PAR file from Eclips to portal. Kindly help me.
