HCM Forms and Process (Displaying Wrong Form )

Hi All ,

I developed 3 diffrent HCM P&F for Transfer, Termination and for Salary Change .

When i am submiting my Salary change form by filling the form in portal , it is dislaying correct form .

and after that i am checking this form by using task->universal worklist and pressing view detail button in portal, it is displaying wrong form ( but displaying correct comment field and value, what i put during submission of the form ) .

I checked work flow as well associated to this process, it is popualting correct form scenario value .

Please give some hint so that i can resolve the issue .
