User Decision Error with Attachment

Dear Experts,

My scenario will upload a PDF attachment as reference for user decision.

Currently, the attachment is successfully created and able to view. And the user decision is work fine before the flow of attachment creation task.

Now, I am getting ERROR status while attachment is being attached to the user decision email, the attachment is still able to view in the email but the selection buttons are gone and show status ERROR.

Anyone facing similar problem? Kindly help..
