User defined form creates dump file for specific user on a purchase order

When we try to enable the UDF form for a Purchase order the system generates a dump file and the window displays a small disconnected UDF form from the main form which I have not seen before. This happens on different window logons at different workstations/server. Is there a setting in the database where these settings are stored per user as the user cannot create PO's due to the fact that the UDF form needs to be used.

As soon as one activates the UDF from the view menu the system creates a dump file and closes with the above explanation of the small size and detached form from main PO form.

We were thnking as a last resort to create another user and that he should use the new user as to carry on with his work.

Any help will be appraciated.
