Monthly IncomeTax Excemption

Hi,

I have a requirement from a cleint wherein they want me to provide a report on excemption considered for a month for following

1. HRA

2. Medical

3. LTA

4. Conveyance.

Is there any standard report for this ? / could any body tell me how do I get this data ( wagetype)?
