The server error occurs in phase PREP_EXTENSION/EHP_INCLUSION.
Last error code set: Enhancement Package inclusion failed, no upgrade package found for component 'EA-APPL'.
All the install files are available.
Any ideas?

check whether EA-APPL 604 was selected in the stack xml file ? go through the xml once.
check if all the EA-APPL 604 packages (EHP4 installation package and Support package) are available in the EPS/in ?
