Adding new fields to ACCOUNT address screen.

I want to add two new fields to one of the Assignment blocks within the Account address screen.

Is this something you would normally do via the Application Enhancement Tool ?.

The reason for asking is that it looks like this tools is only of use if the fields are already created. How does one create those fields in the first place. I also seem to have a problem understanding what tables are behind certain assignment blocks.
