Is there a way to add additional fields to the selection screen for MB5S?

.It is not possible.You need to go for development.Copy MB5S transaction to ZMB5S in Z program selection screen add selection perameters.Check with your technical person.

Would this be and ABAP program change or could it be done with a transaction variant? I know you can hide fields with transaction variants but can you add fields?

It is a standard report.If the report has dynamic selection you can add more fields.But MB5S does not have the option.I would suggest you need to go for new development.
