Lead Creation

Hi experts,

I am working on Campaign management through emails and got stuck at couple of questions.

1) how does a Lead gets created when a customer partcipates the websurvey through the survey link sent to his email id in the campaign management?

Assuming that the customer responded to the survey link hence can you please list the steps in the lead creation process through web survey link.
