Global query output template issue

Hi,

We have a standard template for query output in our production system. When we run queries, some are displaying with as expected template but some queries are displaying the output with blank header.

Have any one faced such issue. Why this global template is not working fine for all queries in common.

Any inputs on this will be appreciated.
