Work item triggered but not in Inbox


I have one doubt regarding workflow for funds reservation. When i create a new FR, standard WF gets triggered and there is one approval step. Problem is even though the agents are properly found, the workitem for approval is not reflected in Inbox. Surprisingly this was not happening 2 days back.

More Info: I tried the following options:

1) SBWP -> Utilities-> Work flow settings -> Refresh Organisational environment.

2) Correctly maintained WLC parameter in the user profile (Tcode SU01 -- Parameters tab)

3) Using SWU_OBUF to refresh runtime buffer.

One more observation is that when i run prog RSWUWFML2 to send notification it is sending properly.

Pls help.
