Customize Benefits Confirmation Form

Hello,

I need to customize the Benefits Confirmation Form (part of the ESS Benefits Participation page). I haven't been able to find this form in the backend. Is it a Smart Form? What is the name?

The Benefits Participation page is in:

Content Provided by SAP -> End User Content -> Employee Self-Service -> iViews -> Benefits and Payment
