A patch, generally speaking, is a way to update an existing piece of software on your computer to a new release.
In the first step, you will have to download patches to be applied. You can download patches for SAP components from the SAP Service Marketplace Software Distribution Center: http://service.sap.com/swcenter.
Once the patch is ready for application, you will require downtime of the system, in which the patch is to be applied & then apply the patches.
