web item in WAD 3.5

Hi ALL,

I have developed a web report. Now my users want an extra notes.......like ..........Prepared BY..... DATE.... and reviewed By........ somewhere on the web report so that they will input values manually take the printout of report.....

So please suggest.....is there any web item to accommodate this?
