Account deletion flag in Detla table

I've had the delta table running for a while with no issues. However about an hour ago it decided to flag a group of accounts as deleted even though they weren't. This, of course, triggered all sorts of pain down the line.

Has anyone seen a simliar issue where the entry in the Delta Table is flagged as Operation = '4' incorrectly?
