AR downpay PLD - Row Type "Text" not "Item" & delete form code

I need either display Item No or Text for Row Type. I did not locate the Row Type for Text that I entered in AR downpayment request. I tried to put two field in the same position for either condition.

Need to delete some form codes in PLD. Did not see "delete" option in PLD.
