fetch data from questionnaire to a smartform?

Hi all,

We have an issue for a complaint transaction. we are filling some required data in the form of questionnaire.

When the complaint is created, we need to send the complaint in smartform through e-mail to the customer, the complaint form should also need to contain data which we filled in the questionnaire.

Can we get the data of questionnaire into a smartfrom .

Please let me know how to proceed?
