SRM Approval Error

Hi All,

We are following Classic Scenario.

When the shooping cart is created, it has to take 4 levels of approval.The third and fourth level approvers will be having the same value limit.

For one of the shopping cart, the fourth level approver rejected the SC, still the follow-on document was created. Can anybody explain what may be the reason for this.
