TRACKING CHANGES TO MOVING AVG PRICE

Hi Guys,

I want to track the changes to Moving Avg Price with in a period. In this tracking report I want For every change in MAP which is the document resposible for that Change and date time etc .

How can I get this.

Please give some solution.
