help - can't view adobe form online

Dear all,

i did a online adobe form and deployed to the server. once i try to run the application..the adobe form won't be shown..any idea? i have no error or what, it displayed the group and other things,just the adobe form itself is not displayed...can anyone kindly tell me what is the problem? thank you.

p/s: i have already configured the ads and install the credentials
