Does anyone have an example of how to extend a Smart Template (List Report) to add another page to the app? This tutorial is useful:

However, it doesn't quite cover adding another page to the app. I assume I will need to create the view and controller, and add the routing to the manifest.json file?
What we're trying to do is add a button to a facet (line item/data fields from another entity set) on the ObjectPage which will open another page with a form that a user can fill out and submit to add another entry to the entityset (write to the database).
