Security issue in SAP BPC 10.0 NW

Hi Experts,

One of our BPC QA system was copied/Refreshed from Production system. Now, when i go to BPC WEB Admin and try to create an user, system says User is successfully added. Please see screen shot below.

When I again go back and click users and check, the user does not exist anymore.

Can you pls. help what is causing the issue?
