Request your expert guidelines in creating 2 appraisal templates:
1) For employees in Managerial role, self appraisal process is followed wherein employees rates himself before his superior (he can be his reporting officer or head of the Department) assesses him. Final review is done by the Divisional / Unit head.
There are three sections in the Appraisal form.
In Section I, rating is to be given out of 20 points for 20 criterias (First Self Apraisal and then by Appraiser).
Section II consisists of provision to enter the strengths and weakness of the employee (Free text entered by appraiser only).
In Section III, Appraiser and Appraisee writes (free text) their own independent comments. Then this Appraisal form is sent to Reviewer (Divisional / Unit head) for his final approval. He only signs and gives his comments.
2) Appraisal of Saff is done by the Controlling Officer (he can be his reporting officer or head of the Department) The Appraisal form is divided into 3 Sections. In Section I, the Appraiser assess the Appraisee on 6 criterias on 4 point scale. In Section II & III, the Appraiser records his comment on the Strengths / Weaknesses of the employee and Training needs respectively
In both cases, the Appraisal forms are then sent to HR Dept.for further followup action (Promotion, Increment etc)
