Purchase Requisition The Header note text field is always non editable

Hi

My requirement is as follows.

Purchase Requisition (MEREQ001): Adding text tab in PR header for approveru2019s/initiatoru2019s remarks as a long text. This long text is appended to header note once the PR is saved. The Header note text field is always non editable.

Please send sample program for this requirement if possible.
