you might want to have a look at the [i.s.h.med site|http://www.ishmed.de/en/html/service/trainings.html]'s training offers, where most of your questions will be answered. I can't supply you the prices, I think they are subject to negotiations. I guess you are welcome to contact the organizers of that trainings at the email address given there.
