Dear,
If we have a material with the following reminder days: 5 - 10 - 20
Then, the first reminder is send 5 days after the statistical delivery date (of course, if we run ME91F)
The second reminder 10 days after the SDD.
The third reminder 20 days after the SDD.
Is it true that the fourth reminder will be send after 30 days of the SDD? If not, when will it be send?

Reminders / Expediter are issued basing on the mentioned interval periods / days in purchasing documents. In case of purchase orders, the messages are issued to Vendors.
In SAP the term u2018reminderu2019 is often used to denote issue messages before due date, where as u2018urging letteru2019 or u2018expediteru2019 commonly stands for a message after due date.
In the system, we can enter up to 3 reminder levels, with positive numbers, negative numbers or combination of both.
A negative number of days means that a reminder is to be issued to the vendor n days prior / before to certain date, positive number of days means that a reminder is to be issued n days after the due date.
As far as I know, there is no 4th reminder level or assumption of 4th reminder automatically basing on previous reminders set - My answer is NO.

Reminders will be sent to Vendor based on your Reminder Days Settings. How System Come to know that the Fourth reminder should be send after 30 days.
There are only Three Reminders in Standard SAP. There is No Fourth reminder. You can Control / Restrict the Vendor with Last Reminder.

I have just tested it, so I can reply on this with the following:
If you have a delivery date of 10/09/2007 and following reminder days set up in the PO:
reminder days 1: 1 day
reminder days 2: 10 days
reminder days 3: 20 days
Then, reminders can be send:
11/09/07 - 1 days after the delivery date
20/09/07 - 10 days after the delivery date
30/09/07 - 20 days after the delivery date
and (so it is not restricted to 3)
20/10/07 - 20 days after the last reminder
09/11/07 - 20 days after the previous reminder
............ - 20 days after each previous reminder
