How to add the "Services for Object" icon on IQS23 (Display Notification)

For people who has a "Services for Object" icon on the IQS23 screen, he can view the workflow overview.

Can someone tell us how to add the "Services for Object" icon onto the upper left hand corrner on the IQS23 screen for (Display Notifcation: PCR with Approval).
