Custom fields not appearing in SUS on po item

We have SRM 5.0. Custom fields are flowing from EBP to SUS via XI, but not appearing on SUS screens. I have seen note 762984 and have the fields attached to the appropriate structures.

I know customizig is required under SRM/Supplier self service /Settings for user interface/Field control settings for tables.

My problem is that I do not understand exactly how to do the table settings. Has anyone done this? Can you give an example of your config setup for a couple of fields? Any hints are appreciated.
