Recover Password task fails

I've created a recover password task that uses two security questions and answers, and I've linked it to the "Recover Password" within password handling.

The user should be able to set a new password when using the task.

When using the task, I enter the MSKEYVALUE of the concerned user, select the Security Questions I had answered, provide the appropriate answers for those questions, then type in my new password and submit! The task keeps failing with a "Could not recover password" error message. When looking at the PHP warnings and notices within the source page I get the following errors:

I've checked and rechecked every detail I can think of but it still continues to fail.

I'd really appreciate any help or suggestions to try and fix the issue!
