material debited twice for Process order

Dear All,

For a single process order of 1 finished good there are 12 individual items.The goods issue for these individual items have been done twice i.e. 2nos of GI documents are done,or material has been issued twice against the same Process Order No.

How this is possible.Where is the problem.

MIGO cannot be done for same process order twice.

During confirmation if there is confirmation twice can this happen.How to check this.Pls can anyone help.
