Looks like the area cost centre was changed after the PR was created that triggered the problem.

It means something terminated the workflow. Looking at your timestamps, it appears to be something within your WF or triggered by it. E.g. an update which raises an event that cancels the WF. Or an event raised in your WF.

From your screenshot it is showing workflow triggered multiple times. Might be you changed mutiple time to trigger workflow multiple time. So as a rule once a workflow for same PR created, other existing workflow instance got logically deleted.
