Getting database totals under a second group

I have a report broken out by doctor, and then procedure. I want to have totals by procedure (for the whole data set) but it keeps breaking it out by the first group. I've tried many different ways but it always comes up as the database total being the same as the doctor total. Any ideas??

Doctor (group 1)

Total By doctor

Database Total

Difference

Dr Smith
