How do I create a TOC with 2 groupings?

I have 2 groups: Category (1) then CustomerName (2). How can I list the Category ....... with associated page number; Then list the Customer Names that belong to that category .... with associated page numbers (while indenting the CustomerName about 5 spaces) ??

Is there a way to create a TOC just using the Crystal Reports application alone (w/o editing the registry, etc.) ?
