CRM - Survey suite - display BP (Customer) information

Good day,

Could someone please advise if it is possible to create a survey. However, on the survey screen it should display the customer name/surname, e-mail and contact number - these fields should also be editable to the customer, and he/she should be able to update these values.

Is this functionality possibility?

We would like to send our customer's a survey, but would also like to ask them check/update their information on the same screen.
